Online Registration Form
INSTRUCTIONS: Please read the following
- The first form is an RSVP form that will notify us about your preferred class and class date(s). There are no transactions made upon filling up this form and will not secure your spot until you fill out the second form below.
- The second form will proceed to check out the class you wish to take and transfer you to PayPal for a secure transaction.
- We will be notified which form(s) you filled. Your emailed receipt from PayPal is your proof and confirmation that you are registered and are attending the class.
- Requalifications, recertifications, and credit for ICC with a DD214 will be available soon. For these classes, you will have to set up an appointment to pay for the classes at our location. Please bring any appropriate documentation for our file and for the discounted rate to be approved.
- Please bring your State issued ID/Driver's license, Firearms Owner ID (FOID) if applicable.
- Please bring something to take notes with (i.e. notebook, black pen).
Class Registration (step 1):
- NO CONFIRMATION EMAIL WILL BE SENT AFTER YOU REGISTER.
- IN ADDITION TO REGISTERING, YOU MUST PAY VIA PAYPAL BELOW. THOSE WHO REGISTER WITHOUT PAYING WILL BE DISREGARDED.
- WALK-INS ARE WELCOME, BUT A SEAT CANNOT BE GUARENTEED. PRE-REGISTRATION IS STRONGLY ADVISED.
Online Check out (Step 2):
NOTE: ALL PAYMENTS ARE FINAL AND THERE ARE NO REFUNDS TO BE ISSUED.
- BY CONTINUING WITH THE PAYMENT PORTION, YOU ACKNOWLEDGE AND AGREE THAT YOU WILL NOT BE REFUNDED IF YOU CANNOT MAKE IT TO THE SCHEDULED DATE.
- BY CONTINUING WITH THE PAYMENT, YOU AGREE THAT YOU CAN ONLY RECEIVE CREDIT TO RESCHEDULE FOR A LATER DATE.
- IF YOU CANNOT MAKE IT ON THE SCHEDULED DATE, YOU WILL BE CREDITED FOR FUTURE CLASSES ONLY.
- Due to high volume of callers and inquiries via email, and our company being small, YOU WILL NOT RECEIVE ANY CONFRIMATION EMAILS FROM US.
- It is your responsibility to contact us if you cannot make it to your scheduled class and to make arrangements for rescheduling for a future date.
- Paypal charges Chicagoland Firearm Training a 2.9% + $0.30 transaction fee. This fee has been passed on to you, the customer.
